Patient charter
Our practice aims to provide each of our patients with a consistently high quality of care. In order to assist us in demonstrating this commitment, we have implemented a Patient Charter to illustrate the standards you can expect.
Essentially we aim to exceed your expectations and also go well above and beyond the fundamental standards you should expect from any healthcare provider. Fundamental standards are defined by the CQC as those under which your care must not fall.
Examples of our recent patient feedback (below) suggests we very often do so.
1. Person-centered care
We aim to provide treatment and care that reflects your needs and is person-centred. This includes explaining your diagnosis and the proposed treatment, as well as how to manage aftercare and help prevent recurrence. If necessary we will with your explicit consent , involve other healthcare professionals such as your GP, Specialists or Consultants collaboratively in your care or refer you to them when appropriate
2. Dignity, repack and courtesy
Expect to be treated with respect regardless of your beliefs, values and preferences without discrimination.
3. Fully informed consent
We understand how important it is for you to understand all the information given to you. Please ask questions if you are unsure of anything. It is important that your Consent is informed. You are free to choose the care you want and should not feel under any pressure. You can take your time when making decisions about your care. We will explain the treatment options open to you, agree a course of action with you and obtain your consent before starting treatment. You may also withdraw your consent to treatment at any time.
4. Costs
The costs you will incur as a result of each visit, consultation and treatment will be clearly explained in advance
5. Safety
We will ensure a safe and comfortable environment for your treatment. This will include ensuring that all premises and equipment used are clean, suitable and properly used. We will do whatever we can to safeguard you from harm of any kind.
6. Collaborative care
We will in accordance with the principles of the Health and Care Act 2022 collaborate only with suitably qualified and appropriate staff and colleagues to ensure that you receive high-quality, advice and care.
7. Complaints
We will provide on request details of our complaints procedure. Any complaint will be properly investigated, and appropriate action taken.
8. Data protection
We will ensure your personal details and health records will be kept strictly confidential. This information will not be disclosed without your written consent unless we are required to do so by law.
9. Duty of candour
We will be open and honest with you should anything go wrong while we are caring for you.
10. Good governance
We strive to have effective governance and systems to check on the quality and safety of care with the aim of service improvement and reducing any risks to your health, safety and welfare. Our track record is excellent and we aim to continue striving for improvement and excellence
11. Staffing
All staff are DBS checked (enhanced) and suitably qualified
12. Display of ratings
We will display any CQC ratings and follow Regulatory guidance
