As well as gaining explicit consent to process and store sensitive personal data in accordance with all data protection regulations and legislation, we are required to ensure consent is obtained for the creation of medical records , for assessment or examination and for any treatment or medical intervention we recommend following a full case history, assessment and medical examination 

Consent to treatment means clear, informed, and voluntary agreement to:

1. Receive specified medical assessment, examination, treatment, or care.

2. Allow healthcare professionals to create, maintain, and use medical records relating to that care.

Under UK healthcare principles, valid consent should be informed, voluntary, and given by a person who has the capacity to make the decision. Consent may be given verbally or in writing, although written consent is commonly used for significant treatments or for documenting explicit agreement. When treating babies informed agreement (consent) must be given by a parent or in some cases both parents with capacity (for example if parents disagree with each other about a treatment or intervention for the baby) 

Explicit Consent for Medical Treatment and Medical Records

 I confirm that I have received sufficient information regarding the proposed medical examination, treatment, or care, including its purpose, benefits, risks, and any reasonable alternatives. One alternative is usually having no treatment or medical intervention.

I also consent to the creation, maintenance, and use of medical records containing information relevant to my healthcare, in accordance with applicable legal, professional, and confidentiality requirements.

I understand that I may withdraw my consent at any time, subject to legal and clinical limitations.

If I am giving consent for the treatment of a minor (under the age of 18 or in some circumstances 16) I confirm that I am the child or infant's parent or legal guardian and have the legal right to consent on their behalf. I agree to provide suitable evidence/identification documents if requested by CalmBaby and will bring ID documents to the initial appointment.
